Two toddlers were injected with heroin by their dad to 'get them to sleep', a report has revealed. The children - and their two older siblings - were found to have suffered 'chronic neglect' at the hands of their parents over a long period of time.
And failings were identified in the way the situation went on to be handled by social services. The harrowing case emerged as part of a now-published Child Safeguarding Practice Review involving several agencies including Lancashire County Council and Lancashire Police.
