A damning dossier reveals junior doctors have worked shifts of up to 21 hours. It lists 15 unsafe staffing incidents a week, including a newly qualified medic left in charge of 44 patients.
The doctors also pointed out two patients died of heart attacks in a corridor awaiting admission to wards. One had waited more than 15 hours.
