Department for Work and Pensions (DWP) to make an initial claim for Personal Independence Payment (PIP), if they meet the basic entitlement conditions relating to age and residence, a ‘How Your Disability Affects You’ form and an information booklet will be sent out by post.

The claimant can use this form to describe how their health condition or disability affects their daily life, on both good and bad days and over a range of activities.
