Werner was expected to leave the Bundesliga side last year but a switch to Bayern Munich collapsed at the last minute. The forward penned a new contract instead but the deal is believed to contain a release clause that could see him depart one year on from putting pen to paper.
The Germany international has been heavily linked with a switch to Anfield and has publicly declared his interest in working with Liverpool boss Jurgen Klopp in recent weeks.
