A prisoner who took her own life a day after seeing her child fall from a window was not appropriately assessed or managed, a scathing watchdog report says.
Christine McDonald, 55, died at Wythenshawe Hospital, a day after being found unresponsive in her cell at Styal Prison in Cheshire.
Ms McDonald was sentenced to 12 weeks custody on March 1, 2019, for two offences of shoplifting shampoo, bubble bath, hair dye and cheese and one offence of failure to comply with a community requirement of a suspended sentence.
During her arrest in Blackpool, earlier that day, Ms McDonald, who was withdrawing from drugs, had witnessed her child fall from a third-floor window.
