A man took his own life just 11 days after his release from prison. Robert Connolly, who was 46, was found dead at his home on April 29 last year, a newly-published report into his death has revealed.

He had only just been released from HMP Featherstone in Staffordshire. The Prisons and Probation Ombudsman, which independently investigates deaths in custody, also looks into post-release deaths occurring within 14 days of a person's release from prison.

The report by ombudsman Adrian Usher says Mr Connolly, who had a history of substance misuse, 'completed a detoxification programme in prison and received appropriate support'.

Prior to his release he was also referred to Turning Point, a Rochdale-based community drug and alcohol team. Mr Connolly, says the report, was remanded in custody on May 18, 2023, charged with burglary.
