The Manchester Evening News understands the "significant concerns and rectification notice" was slapped on the jail two weeks ago by the Ministry of Justice.
The privately run prison is operated by Sodexo Justice Services and if progress is not made the company could lose the contract which is scheduled to run until 2025.
