Coroner concerned over Royal Oldham Hospital's admissions assessments following death of man, 84

Julie Robertson, the coroner for Manchester North, has warned that there is a "risk that future deaths will occur unless action is taken".

A spokesperson from the hospital has said that it will "implement any actions required to improve patient safety". A nurse carried out an admissions assessment on Leslie Horsfield, from Failsworth, when he arrived in A&E on October 1, 2020.

This included establishing whether he had any swallowing difficulties. However, the nurse did not ask him directly if he had experienced episodes of choking in the past and he did not volunteer that
