A four-year-old boy died after he got trapped between two stair gates in a "tragic accident" at home, which police said was in "absolute squalor".
The little boy, referred to in a serious case review as AH, was found just days after a health visitor had raised concerns over the "filthy" property and the risk to the child and others at the home.
Police who attended the home in Norfolk said it was in "absolute squalor". The house was described as "cluttered with an unpleasant odour from the stained carpets and piles of unwashed clothing", with 45 bags of dirty washing removed by a cleaning company.
