James Thornton-Chan, 22, died at a house on Stockport Road in Manchester on July 14, 2019. A toxicology report found the presence of dihydrocodeine in his system 'at a level which would explain his death'.

Mr Thornton-Chan, who was born in the High Peak in Derbyshire, was prescribed codeine after complaining of back pain. He attended his GP practice in Bramhall in July 2018 and told a doctor he had taken a number of codeine tablets over the previous 24 hours as a result of the pain he was
