Fernandes sustained an ankle injury in the first half of United's FA Cup semi-final win over Brighton but continued to play for the full 90 minutes, before being substituted ahead of extra-time.
Pictures of the midfielder in a protective boot intensified concerns that the injury could rule him out for the rest of the season.
When asked for an update on Fernandes, Ten Hag admitted that the Portuguese ace is a "question mark" for Thursday's Premier League clash with Spurs.
