Two investigations are underway into the death of a baby in Tameside General Hospital earlier this year. Luca Yates tragically passed away at just one-day-old in the hospital on January 24 this year.

Today, a pre-inquest review into her untimely death revealed that two investigations into the circumstances are underway. Both of those fall alongside the coronial case, which is being presided over by Area Coroner Chris Morris.

The two investigations are being undertaken by the NHS Trust which runs Tameside General Hospital and by the Healthcare Safety Investigation Branch (HSIB), an independent body. She told the court: “This process is to look at the medical records and interview staff involved in the care of Miss Ditchfield and also uses clinical advisors to help us prepare the report that are independent of the Trust.
