Michael McDonagh, 27, was found unresponsive at HMP Forest Bank in February 2019. An inquest heard the dad-of-one had been taking a number of medications to treat mental health problems including schizophrenia, anxiety, and depression, as well as substance abuse issues.

"I don't want any other family to go through what we have experienced," she added. "I'm grateful every day for the time we got to share with him." Giving evidence, pathologist Dr Naomi Carter said post mortem examinations showed Mr McDonagh died from bronchopneumonia after a 'depressant' of his nervous system, which was likely caused by a 'combination' of medications.
