A dad who tragically took his own life had asked to be hospitalised, an inquest heard. "Loving dad, son and brother" Scott Pharo was discovered dead in October 2019 after enduring a year of mental health difficulties.

A conclusion of suicide was recorded by the area coroner Richard Taylor during an inquest which was held at Accrington Town Hall in Accrington, Lancs Live reports.Scott was originally from Wakefield but at the time of his death, he lived in Clayton-Le-Moors with his wife Caroline and their three children.Throughout the inquest, Caroline expressed concern that Scott's requests to be taken to hospital were not heard before his death.However, Dr Kenneth Han, of the Daisyfield mental health service, and Nick King, of Lancashire & South Cumbria NHS Foundation Trust (LSCFT), both gave assurances that the correct procedures were followed.Dr Han explained that he had seen Scott for an initial appointment and concluded that he would benefit from new medication and visits from the home service team three times a week.

He explained that this was the most severe intervention other than hospitalisation.The doctor also explained that while Scott had discussed having suicidal thoughts, he had shown no recent intent of killing himself.He added: “When I saw him, he said he did not want to kill himself.

However, because of his history, I thought it appropriate to keep him under close contact.”Giving evidence of LSCFT’s investigation, Mr King said he did not believe that those involved in his treatment has missed any warning signs but warned that pressures on mental health services nationwide had changed the way some cases are handled.Speaking generally about mental health treatment, he said: “When I was practising as a
