A troubled mother took her own life in woodland a day after a community nurse concluded she was 'stable', an inquest heard. Julie Ainsley, 47, who with a history of severe mental health problems and suicidal thoughts, had been sectioned under the Mental Health Act and discharged three months before her death.
The mum-of-four, who had previously attempted to jump from a bridge and the roof of a car park, was under the care of a community mental nurse who was said to have failed to carry out a risk assessment on the patient when she met her 24 hours before the tragedy.
