The devastating case is one of 700 incidents across NHS England in which basic errors led to tragic outcomes, a Freedom of Information Act survey by the Sunday Express found.

Surgeons carried out the operation in order to try and save the healthier twin when it was discovered that its sibling had restrictive growth, increasing the chances of stillbirth and complications.
